To be more specific the portal is located where the ending of the Dark Monastary mission takes place.You also have to follow Toland back to the exit and take the path his little orb takes (right vs left). You can drop the sword and pick it back up but do need to take it all the way to the exit to finish the activity. I once had to do that realm three times in a row due to the quest not clearing properly, and that was well over a year ago. c': 4.
