Phone numbers that are not programmed in as contacts can get through, but numbers that are saved in her contacts are getting automatically routed to voicemail. This is something that Verizon says they have never seen before. Starred and emergency contacts cannot get through regardless of their status.

Phone > Settings > Voicemail > Visual Voicemail. The voicemail icon will only appear right when the voicemail is left. If you don't look at it quickly, it goes away and only comes back on a restart.
